I'm not picky, I don't really care as long as it's not other recordings (cues, concert pieces) slopped together and there's been some thought put behind it. I think Indy 4, for example, has an excellent end credits piece.

Definitely option 3. It's cool to revisit all the familiar themes, but hearing them in a new way keeps things fresh.

JNH does this a lot. Several of his End Title pieces contain either completely new material sandwiched in between stuff from the movie, or rerecordings/ alternate /film takes of existing themes. Many others are clearly specifically recorded pieces. He also seems to be releasing more of them on CD (I know I'm gushing, but I've made no secret of how well made I think most JNH albums are).

Everything. It depends on the movie. I don't think end credits "suites" are always the right option; sometimes movies end with a sense of closure that negates the score's early themes, particularly villain themes (in this regard, I don't like the credits music for The Final Conflict, the film version of The Phantom of the Opera or Kingdom of the Crystal Skull).

I don't care. I care more about how the actual credits are presented after the movie finishes. The amount of pause before the first title appears is extremely important, it varies depending on the type of ending you have, but something like the ending to X-Men: The Last Stand is terrible. I also don't like when it starts to scroll immediately. I think you should always have the main credits up in slideshow first, then roll after the main people are shown. That style also allows for a better scoring style; think Ratatouille, The Incredibles, Lemony Snicket, etc.
